Effects of fungal supplementation on endurance, immune function, and hematological profiles in adult athletes: a systematic review and meta-analysis
Ultra Blend uses a Cordyceps sinensis 4:1 extract. The pooled analyses in this review examined Cordyceps sinensis preparations, so the species matches, though the specific preparations varied between the included trials.
The review set out to pool randomised controlled trial data on fungal supplementation in adult athletes across endurance performance, immune function and haematological outcomes, with results stratified by fungal species.
Four databases were searched for randomised controlled trials in adult athletes. Methodological quality was appraised, and outcomes were pooled with assessment of statistical heterogeneity and analysis stratified by species.
For the Cordyceps sinensis subgroup the authors reported statistically significant improvements against placebo in ventilatory threshold, peak oxygen uptake and endurance performance, with low statistical heterogeneity between pooled trials.
The Cordyceps subgroup pools only two to three small trials per outcome and the reported p-values sit close to the conventional significance boundary, so a single further null trial could alter the result. Confidence intervals are not reported for every outcome. The pooled trials were conducted in athletes and do not describe untrained populations.
Selected because it sits at the top of the evidence hierarchy for this ingredient and is the only synthesis restricted to single-species Cordyceps preparations, with blend products excluded from the pool.

Effect of Cs-4 (Cordyceps sinensis) on exercise performance in healthy older subjects: a double-blind, placebo-controlled trial
Ultra Blend uses a Cordyceps sinensis 4:1 extract. This study used Cs-4, a fermented mycelial preparation of the same species, so the species matches while the preparation differs.
The study investigated whether twelve weeks of Cs-4 supplementation affected exercise performance in healthy older adults, assessed by maximal incremental cycle ergometry with breath-by-breath gas analysis.
Participants completed a maximal incremental exercise test on a stationary cycle ergometer at baseline and again at twelve weeks. Outcome measures were maximal oxygen uptake, the metabolic (lactate) threshold and the ventilatory threshold.
The authors reported a 10.5% rise in the metabolic threshold and an 8.5% rise in the ventilatory threshold in the group receiving Cs-4, with no corresponding change in the placebo group. Maximal oxygen uptake was unchanged in both groups.
A small pilot sample of twenty adults aged 50 to 75, so the findings may not extend to younger or trained populations. The reported statistics are within-group before-and-after comparisons rather than between-group treatment effects, which limits how strongly the difference from placebo can be stated. Results should be interpreted cautiously and further research is required.
Selected because it is the clearest randomised, placebo-controlled trial of a single-ingredient Cordyceps preparation using objective gas-exchange measurement rather than self-report.

Cordyceps sinensis (CordyMax Cs-4) supplementation does not improve endurance exercise performance

The study investigated whether five weeks of Cordyceps supplementation affected peak oxygen uptake, ventilatory threshold and endurance performance in trained cyclists.
Twenty-two endurance-trained male cyclists were allocated to 3 g per day of the Cordyceps preparation or placebo for five weeks under double-blind conditions, with training documented weekly. Participants completed a graded exercise test and a work-based time trial before and after supplementation.
No statistically significant difference was observed between groups for peak oxygen uptake, ventilatory threshold or time-trial performance. The authors concluded that five weeks of supplementation did not affect aerobic capacity or endurance performance in this population.
A small sample of twenty-two participants over five weeks, limiting the ability to detect modest effects. The cohort was exclusively endurance-trained males, so the result does not describe untrained adults, women or older adults. A single preparation at one dose was tested.
Included because the evidence for this ingredient is mixed, and presenting only supportive findings would misrepresent it. This trial is the methodological counterpart to the positive studies above and reports a null result in trained athletes.

Benefits, side effects, and uses of Hericium erinaceus as a supplement: a systematic review
The review set out to collate the published evidence on the reported effects, adverse effects and applications of Hericium erinaceus taken as a dietary supplement, across cognitive, gut and mood-related outcome domains.
The authors searched the literature following PRISMA guidance, with the protocol registered in advance on PROSPERO. Twenty-six studies met the inclusion criteria, and the review was restricted throughout to Hericium erinaceus as a single agent.
The authors reported that the included clinical studies described improvements in cognitive measures, and that laboratory studies described effects on gut microbiota diversity and on markers associated with neurogenesis. Documented adverse effects across the literature were limited to stomach discomfort, headache and allergic reactions.
Most included records were laboratory rather than human studies, and only five randomised controlled trials were identified. No quantitative meta-analysis or pooled effect estimate was performed, so the strength of the human evidence cannot be judged from the synthesis alone. The search was limited to a single database.
Selected because it is the most recent prospectively registered systematic review restricted to this ingredient as a single agent, and because it documents adverse effects alongside reported benefits.

Improving effects of the mushroom Yamabushitake (Hericium erinaceus) on mild cognitive impairment: a double-blind placebo-controlled clinical trial
The study investigated whether sixteen weeks of oral Hericium erinaceus affected cognitive function, measured on a scale based on the Revised Hasegawa Dementia Scale.
Participants were randomised to Hericium erinaceus tablets or placebo for sixteen weeks under double-blind conditions, with cognitive assessment at four-weekly intervals and a further four weeks of observation after intake ceased.
The authors reported that participants receiving Hericium erinaceus scored significantly higher on the cognitive function scale than placebo at weeks 8, 12 and 16, and that scores rose with duration of intake. Scores declined significantly in the four weeks after intake stopped. Laboratory testing indicated no adverse effect attributable to the intervention.
A small sample of fifteen participants per arm drawn from a single national population. Participants had diagnosed mild cognitive impairment, so the findings do not describe healthy adults. A single cognitive instrument was used, with no biomarker or imaging endpoints, and the difference did not persist once supplementation stopped.
Selected because it is the most widely cited randomised, double-blind, placebo-controlled trial of this ingredient given as a single agent, and because its follow-up period shows what happened after intake ended.

The acute and chronic effects of lion's mane mushroom supplementation on cognitive function, stress and mood in young adults: a double-blind, parallel groups, pilot study
The study investigated both the acute single-dose and the 28-day effects of Hericium erinaceus on cognitive performance, subjective stress and mood in healthy, cognitively unimpaired adults.
Participants were randomised in a double-blind, parallel-groups design to 1.8 g per day of Hericium erinaceus or placebo for 28 days. Cognitive performance was assessed at intervals after the first dose and again at day 29, using the Stroop task, word recall, a global cognition composite, the Perceived Stress Scale and Visual Analogue Mood Scales.
The authors reported significantly faster Stroop task performance 60 minutes after a single dose. No statistically significant difference was observed on the global cognitive composite or on most individual tasks, and reduced accuracy on immediate word recall was observed acutely. At 28 days no statistically significant differences were observed on the stress or mood scales.
An explicitly designated pilot study with a small sample, and the authors state the findings should be interpreted with caution. Multiple outcomes assessed at several timepoints raise the possibility of chance findings, and one memory measure moved in an unfavourable direction. The population was young and healthy over a short 28-day period.
Included because the evidence for this ingredient is mixed, and because this is the best-designed trial in healthy younger adults. Its predominantly null results show that findings from cognitively impaired participants have not been reproduced in a healthy population.

Does ashwagandha supplementation have a beneficial effect on the management of anxiety and stress? A systematic review and meta-analysis of randomized controlled trials
The review evaluated the pooled effect of ashwagandha supplementation on anxiety and stress outcomes across the randomised trial literature, and examined whether a dose-response relationship was present.
Three databases were searched from inception to December 2021 for randomised clinical trials of ashwagandha extract on anxiety and stress. Twelve eligible trials totalling 1,002 participants were pooled using a random-effects model, with a non-linear dose-response analysis and formal appraisal of the certainty of evidence.
The authors reported statistically significant reductions in both anxiety and stress measures compared with placebo. Statistical heterogeneity between trials was very high for both outcomes, and the authors graded the certainty of the evidence as low, concluding that further high-quality studies are required.
Heterogeneity between trials was very high, so the pooled estimates may not describe any single population, dose or extract. The authors themselves rated the certainty of evidence as low. Many constituent trials were small and manufacturer-funded, and the dose-response model extends well beyond typical supplemental intakes.
Selected because it aggregates the randomised evidence rather than resting on any single trial, and because it grades its own certainty openly, which allows the strength of the evidence to be judged.

An investigation into the stress-relieving and pharmacological actions of an ashwagandha (Withania somnifera) extract: a randomized, double-blind, placebo-controlled study
The study investigated the stress-relieving effects of a standardised ashwagandha extract in adults reporting high stress, and examined the hormonal changes accompanying them.
Sixty stressed but otherwise healthy adults were randomised to 240 mg of a standardised extract once daily or matched placebo for sixty days under double-blind conditions. Outcomes were the Hamilton Anxiety Rating Scale and the DASS-21, alongside morning cortisol, DHEA-S and testosterone. The trial was prospectively registered.
All sixty participants completed the trial with no adverse events recorded. Participants receiving ashwagandha showed a statistically significant reduction in Hamilton Anxiety Rating Scale scores relative to placebo, and significantly greater reductions in morning cortisol. The reduction on the DASS-21 did not reach statistical significance.
A small single-site sample over sixty days. The primary between-group anxiety difference was marginal while a second validated scale returned a null result. The study was funded by the manufacturer of the extract tested, and much of the wider literature on this ingredient originates from a narrow set of research groups.
Selected because it combines double-blind placebo control, prospective registration, complete follow-up with no dropouts, and objective hormonal measurement alongside validated psychometric scales.

Randomized placebo-controlled adjunctive study of an extract of Withania somnifera for cognitive dysfunction in bipolar disorder
The study investigated whether eight weeks of a standardised Withania somnifera extract, added to existing stable medication, altered cognitive performance, alongside mood stability and tolerability.
Participants maintained on stable medication were randomised in a double-blind fashion to 500 mg per day of the extract or placebo for eight weeks. A battery covering working memory, attention and social cognition was administered with mood and anxiety rating scales. Primary funding came from an independent research foundation.
The authors reported a statistically significant advantage for the extract on three of the cognitive tasks assessed, with a medium effect size on one. No statistically significant differences were observed on the remaining cognitive measures, and mood and anxiety scores remained stable in both groups. Adverse events were described as minor.
Only three of a large number of cognitive comparisons reached significance, so chance findings from multiple testing cannot be excluded. Attrition was uneven between groups, the sample was small, and participants were a medicated clinical population rather than healthy adults. Study material was supplied by the extract manufacturer.
Included because it is independently funded and academically led, which counterbalances the largely manufacturer-funded literature for this ingredient, and because its mostly null results show the evidence is not uniform.

Clinical and preclinical systematic review of Panax ginseng C. A. Mey and its compounds for fatigue
Part of this review synthesises animal studies. Those analyses are preclinical and do not describe outcomes in people.
The review set out to synthesise the randomised clinical trial evidence on Panax ginseng in relation to fatigue, and to summarise the biochemical mechanisms proposed across the supporting animal literature.
Seven English and Chinese language databases were searched from inception to August 2019. Trial quality was appraised with the Cochrane risk-of-bias checklist and animal studies with the CAMARADES checklist, and data were pooled using standard meta-analytic software.
The authors reported pooled results favouring Panax ginseng over placebo on the fatigue scales used in the included trials and on a composite clinical effect measure. No statistically significant difference in adverse effects was observed between ginseng and placebo groups.
The clinical evidence base is small at eight trials, and risk-of-bias scores indicate several had methodological shortcomings. Included trials differed in population, preparation, dose and outcome instrument, so pooling across heterogeneous fatigue scales limits interpretation. A substantial part of the analysis rests on animal data, which does not establish clinical outcomes in humans, and the authors' concluding recommendation is stronger than the trial evidence supports.
Selected because it is the synthesis restricted to Panax ginseng specifically rather than the wider genus, and because it applies formal risk-of-bias appraisal to every included study.

Anti-fatigue effects of Korean Red Ginseng extract in healthy Japanese adults: a randomized, double-blind, placebo-controlled study
The study investigated whether Korean Red Ginseng extract reduces fatigue in healthy adults over three weeks, alongside a full safety assessment.
Forty-six healthy adults were randomised to Korean Red Ginseng extract or matched placebo for three weeks under double-blind conditions. Fatigue was assessed by visual analogue scale, with plasma lactic acid and cortisol as objective markers, and safety monitored through adverse events, urinalysis, haematology and blood biochemistry.
The authors reported significantly lower fatigue scores in participants receiving Korean Red Ginseng than placebo. No statistically significant difference was observed between groups for plasma cortisol or lactic acid, so the effect was seen in self-rated fatigue rather than in those blood markers. Safety measures were comparable to placebo.
A small sample over a short three-week period. The benefit was confined to a self-reported measure while both objective fatigue markers returned null results, so results should be interpreted cautiously. The study was published in a journal closely associated with the ginseng industry.
Selected because it examines fatigue in healthy adults rather than a clinical population, which is the more relevant comparison for most readers, and because it is the most recent trial of its kind.

A double-blind, randomized, placebo-controlled trial of Panax ginseng for cancer-related fatigue in patients with advanced cancer
The study set out to determine whether a standardised Panax ginseng extract was superior to placebo for fatigue, with quality of life, mood and symptom burden as secondary endpoints.
Patients reporting moderate or greater fatigue were randomised to 400 mg of Panax ginseng extract twice daily or matched placebo for 28 days. The primary endpoint was change in a validated fatigue subscale, with quality of life, anxiety, depression and functional scores assessed as secondary endpoints.
No statistically significant difference was observed between groups on the primary fatigue endpoint. Both groups improved significantly from baseline, indicating a substantial placebo and regression-to-the-mean component. Secondary outcomes covering quality of life, mood and function were also not significantly different between groups.
Conducted in a single clinical population over 28 days, so the null result does not describe healthy adults, other doses or longer exposures. Fatigue was measured by self-report instruments sensitive to expectancy effects, and the marked improvement in the placebo group limits the sensitivity of the comparison.
Included because it is the largest well-controlled trial of this ingredient for fatigue and returned a clearly null primary result. Presenting it alongside the positive findings above is necessary for the evidence to be represented fairly.

Meta-analysis of randomized controlled trials on cognitive effects of Bacopa monnieri extract
The review set out to identify and quantitatively pool randomised, placebo-controlled trials of chronic standardised Bacopa monnieri extract, and to establish which cognitive domains showed consistent effects across trials.
Randomised, placebo-controlled trials of standardised extracts given for at least twelve weeks without co-medication were included. Nine trials totalling 518 subjects met the criteria, with quality appraised using the Cochrane risk-of-bias tool and the Jadad scale.
The pooled analysis reported significantly shortened Trail Making Test B completion times and significantly decreased choice reaction times relative to placebo. The authors concluded that the evidence points towards an effect on speed of attention, while emphasising that larger and more rigorously designed comparative trials are required.
Nine trials and 437 pooled subjects is a small evidence base, and the included trials varied in extract preparation and population. Effects concentrated on a narrow set of attention-speed measures rather than across cognitive domains. The authors state that a large, well-designed comparative trial is still needed.
Selected because it is the principal quantitative synthesis restricted to chronic single-ingredient trials of this extract, and pools the primary evidence rather than relying on one study.

Examining the nootropic effects of a special extract of Bacopa monniera on human cognitive functioning: 90 day double-blind placebo-controlled randomized trial
The study investigated whether ninety days of a standardised Bacopa monnieri extract altered cognitive performance in healthy adults relative to placebo, using a validated computerised battery.
Healthy adults were randomised in a double-blind, placebo-controlled independent group design and received the extract or matched placebo daily for ninety days. Cognitive assessment was conducted at baseline and after the treatment period using the Cognitive Drug Research computerised system.
The authors reported significantly improved performance on the working memory factor, specifically spatial working memory accuracy, in participants receiving the extract. A reduction in false-positive responses on a rapid visual information processing task was also reported. The authors noted that the effective dosage range and the effects of longer administration remain to be established.
Substantial attrition, with 62 of 107 recruited participants completing, which reduces statistical power and raises the possibility of differential dropout. Effects were confined to a subset of measures within a broad battery, so multiple-comparison inflation cannot be excluded. A single proprietary extract at one dose was tested in healthy volunteers.
Selected because it is among the largest chronic, double-blind, placebo-controlled trials of this ingredient in healthy adults and uses a validated computerised battery with a pre-specified endpoint.

Brahmi for the better? New findings challenging cognition and anti-anxiety effects of Brahmi (Bacopa monniera) in healthy adults
The study set out to assess the effects of twelve weeks of Bacopa monnieri extract on learning, memory, information processing and anxiety in healthy adults, noting that inconsistent results in the existing literature may relate to methodological differences.
Seventy-two healthy adults were randomised in a double-blind, placebo-controlled parallel-group design to 450 mg per day of the extract or matched placebo for twelve weeks. Outcome measures covered verbal learning and memory, inspection time, attention and interference, alongside state and trait anxiety.
No statistically significant difference was observed between the extract and placebo groups on any of the cognitive measures assessed. The authors reported a non-significant trend towards lower state anxiety, and framed the overall findings as challenging earlier reports of cognitive and anti-anxiety effects.
With 72 participants across two groups the trial may have been underpowered to detect small effects, so a null result does not establish the absence of an effect. A single dose over a single twelve-week period was tested in healthy, non-impaired adults, which may limit sensitivity to change.
Included because it is a well-controlled trial reporting a null result on the same outcome domains as the positive studies above. Showing it makes clear that the findings across this literature are not uniform.

Effects of maca (Lepidium meyenii Walp.) on physical performance in animals and humans: a systematic review and meta-analysis
Most of the studies pooled in this review are animal experiments. Those analyses are preclinical and do not describe outcomes in people.
The review set out to examine studies in which maca supplementation was used with the aim of increasing physical performance, in both animal and human experimentation, and to calculate the associated effect sizes.
The authors followed PRISMA guidance and searched five databases to 2024, including randomised controlled studies with a pre- and post-test design. Quality was appraised with the CAMARADES tool for animal studies and the Newcastle-Ottawa Scale for human studies, and outcomes were pooled as standardised mean differences.
The authors reported large pooled effect sizes across the analysed performance outcomes and concluded that a dose-response relationship was apparent. The forced swimming, rota-rod and grip strength outcomes that dominate the pooled analysis are animal models.
Only four of the twenty-one included studies were human trials, so the pooled effect sizes are driven largely by animal experiments and do not translate directly to human performance. Heterogeneity across included studies was high, and the authors note that the proposed mechanisms require conclusive evidence in humans. Cultivars and preparations differ across the pooled studies.
Selected because it is the synthesis restricted to physical-performance and fatigue outcomes, and because the balance of animal to human studies within it is itself informative about where this evidence base currently stands.

Acceptability, safety, and efficacy of oral administration of extracts of black or red maca (Lepidium meyenii) in adult human subjects: a randomized, double-blind, placebo-controlled study
The study investigated the effects of spray-dried red and black maca extracts on self-reported mood, energy and health-related quality of life, alongside blood glucose, blood pressure and haemoglobin, with acceptability and safety as secondary outcomes.
Participants were randomised in a double-blind, placebo-controlled design to 3 g per day of placebo, red maca extract or black maca extract for twelve weeks. Outcomes were assessed by questionnaire alongside blood glucose, blood pressure and haemoglobin measurement, with adverse effects self-recorded throughout.
The authors reported that participants receiving red or black maca recorded higher mood, energy and health status scores than placebo, with the larger differences reported for red maca. Both extracts were well tolerated with no serious adverse effects recorded.
Mood and energy outcomes were self-reported questionnaire measures rather than objective performance tests. The published abstract contains an internal inconsistency between the stated participant total and the reported group allocations. The cohort comprised Peruvian lowland and highland residents, and cultivars differ considerably in composition, which limits comparison with other trials.
Selected because it is among the largest and longest placebo-controlled trials of this ingredient and reports mood and energy outcomes, and because it compares two cultivars directly.

A pilot investigation into the effect of maca supplementation on physical activity and sexual desire in sportsmen
The study investigated whether fourteen days of maca extract supplementation altered endurance cycling performance in trained male cyclists compared with placebo.
Eight experienced male cyclists completed 40 km cycling time trials before and after each of two fourteen-day supplementation periods in a randomised, double-blind crossover design with a washout of at least one week. Time-trial performance was the principal physiological outcome.
Participants receiving maca improved their 40 km time-trial performance relative to their own baseline. However, no statistically significant difference was observed between the maca and placebo conditions after supplementation, so the improvement did not exceed that seen with placebo.
A very small sample of eight participants, described by the authors as a pilot investigation, so it is underpowered and the null between-group result cannot be treated as evidence of no effect. Two authors were affiliated with the company supplying the extract, and the cultivar used was not specified.
Included because it is the only trial of this ingredient with a direct exercise-performance endpoint, and because it illustrates a pattern common across nutrition research, where a within-group improvement does not survive comparison against placebo. The trial's second outcome concerned sexual desire, which is outside the scope of this page and is not discussed here.
